Hospitals in Washington Earn ‘A’ on the Spring 2025 Hospital Safety Grade
The Leapfrog Group, an independent national nonprofit focused on patient safety, released its Spring 2025 Hospital Safety Grade Thursday, showing that Washington had slipped in its rank among states with the highest percentage of “A” grades.
Previously ranked 17th last fall, this latest iterations of grading drops Washington down to 29th overall in the United States with fewer than 1 in 3 hospitals receiving an “A” grade.
The biannual Safety Grade is an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries and infections. Driven in Washington by the Washington Health Alliance, the grades give the community critical insight about local hospitals.
In Washington, 47 hospitals were graded, and 12 earned an A, 13 earned a B, 20 earned a C, and two earned a D. No Washington hospitals received an “F” in this latest round of grading.
For the first time, Leapfrog is spotlighting the 346 hospitals nationwide that not only achieved an A for spring 2025 but sustained the grade for more than two years. This select group, representing 12% of all eligible hospitals, is now highlighted on www.hospitalsafetygrade.org.
The following hospitals in Washington are among those Straight A hospitals:
- Virginia Mason Medical Center in Seattle
- PeaceHealth St. Joseph Medical Center in Bellingham
- PeaceHealth Southwest Medical Center in Vancouver, WA
- EvergreenHealth in Kirkland
- St. Michael Medical Center in Silverdale
Virginia Mason Medical Center holds the distinction of being one of only 11 hospitals nationwide to maintain an “A” in safety since 2012.

The Leapfrog Hospital Safety Grade is the only hospital ratings program focused exclusively on preventing medical errors and patient harm. It is updated biannually in the fall and spring, offering the public a reliable, independent assessment of hospital safety. The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel.
